Due to an increase in the number of referrals we are receiving, Fair Ways is looking to recruit additional Outreach Support Workers to work in partnership with families and carers of service users, in achieving the aims and objectives of care-plans/in house placements.

What you will be doing
The successful candidate will be required to;
- Complete daily contact sheets as well as promoting and safeguarding our Service Users welfare.
- Support and assist service users with personal care tasks where necessary and be proactive in planning and carrying out fun activities in the community.
- Recognise the need to intervene whilst managing challenging behaviour resulting from complex needs.
- Observe, assess and monitor Service User’s health development and emotional well-being.
- Regularly review and update Risk Assessments and Care Plans in line with the needs of the service user.
- Attend team meetings, partake in handovers and provide positive and informative critique.
What we want from you
Ideally, we are looking for male candidates to act as role models to our male service users, but we are open to applications from both male and female candidates. Above all, you need to be someone who cares, who wants to make a difference and is a reliable and tenacious individual. Ideally you will have experience of working with children, young people and/or adults with both learning and physical disabilities, however this is not essential as full training will be provided. Holding a Level 2/3 Diploma in Health and Social Care or equivalent is desirable, but not essential to the post.
You will need to have a full driving license and have access to the use of a car. Successful candidates will be required to provide an enhanced Disclosure (DBS). This disclosure cost will be met by Fair Ways.
